When to use it
- A patient is due for their annual wellness visit (AWV) or annual physical
- Running a preventive care outreach campaign for a specific patient population
- A patient has an overdue lab order or screening (colonoscopy, mammogram, cervical cancer screening)
- Chronic disease patients (diabetes, hypertension) need routine follow-up reminders
- Pediatric patients due for vaccination catch-up
- Closing care gaps before year-end (before insurance resets or deductibles reset)
- Tracking preventive care completion rates for value-based care contracts
Mistakes to avoid
- Sending mass reminders without segmenting by patient preference — some patients have opted out of SMS or email; ignoring this creates compliance violations and frustrates patients
- Using the wrong CPT code for tracking — if the AWV was billed under a problem-oriented E&M code instead of the preventive care code, the EHR may show the patient as "not compliant" when they actually are
- Forgetting to re-recapture the patient after a refused service — if a patient declined a flu shot in October, they should be offered again in November (new batch, new supply); don't treat a refusal as permanent unless the patient explicitly opts out permanently
- Not tracking referrals generated from care gap outreach — sending a patient for a colonoscopy referral doesn't count as "colonoscopy completed" until the procedure is actually done; follow up on every referral sent
